Key Responsibilities:
• Provide high-quality nursing care to residents in a long-term care and skilled nursing facility setting • Develop, implement, and evaluate individualized care plans to meet the unique needs of each resident • Collaborate with interdisciplinary teams to coordinate resident care and achieve optimal outcomes • Monitor vital signs, administer medications, and perform other nursing duties as needed • Maintain accurate and up-to-date resident records and reports
Required Qualifications:
• Current licensure as a Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) in the state of NM • Basic Life Support (BLS) certification • Ability to lift, transfer, and position residents as needed
Preferred Experience / Skills:
• Experience working in a long-term care or skilled nursing facility • Knowledge of gerontological nursing principles and practices • Ability to communicate effectively with residents, families, and healthcare teams
