LVN / LPN LTC/SNF
Job Title: LVN / LPN - LTC/SNF
Location: Albuquerque, NM
Schedule: Full-time, Days
Key Responsibilities:
• Provide high-quality nursing care to residents in a long-term care setting • Develop and implement individualized care plans to meet the unique needs of each patient • Collaborate with interdisciplinary teams to coordinate patient care and achieve optimal outcomes • Monitor vital signs, administer medications, and perform various medical procedures as needed • Educate patients and families on health promotion, disease prevention, and treatment options
Required Qualifications:
• Current licensure as a Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) in the state of NM • 1+ year of experience in long-term care or skilled nursing facility (SNF) setting • Ability to lift, transfer, and position patients as needed • Basic life support (BLS) certification
Preferred Experience / Skills:
• Experience working in a long-term care or SNF setting • Knowledge of medical terminology and anatomy • Ability to communicate effectively with patients, families, and interdisciplinary teams
