LVN / LPN LTC/SNF

Job Title: LVN / LPN - LTC/SNF

Location: Lebanon, NH

Schedule / Job Type: Full-time, Days

Key Responsibilities: • Provide high-quality patient care in a long-term care setting, including administering medications, performing vital signs, and documenting patient information. • Develop and implement individualized care plans to meet the unique needs of patients, including those with complex medical conditions. • Collaborate with interdisciplinary teams to coordinate patient care and ensure seamless transitions. • Educate patients and families on health promotion, disease prevention, and management strategies. • Maintain accurate and up-to-date patient records, including medical history, treatment plans, and progress notes.

Required Qualifications: • Current licensure as a Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) in the state of New Hampshire. • Valid certification in basic life support (BLS) and/or advanced life support (ALS). • Ability to lift, transfer, and position patients as needed.

Preferred Experience / Skills: • Experience working in long-term care or skilled nursing facilities. • Knowledge of gerontological nursing principles and practices. • Bilingual or multilingual skills, as desired.
