Key Responsibilities: • Provide high-quality patient care in a long-term care facility or skilled nursing facility setting • Assess, plan, implement, and evaluate patient care plans to meet individual needs • Collaborate with interdisciplinary teams to coordinate patient care and achieve optimal outcomes • Administer medications, treatments, and procedures as prescribed by physicians • Educate patients and families on health promotion, disease prevention, and management techniques
Required Qualifications: • Current licensure as a Registered Nurse in the state of NH • Experience working in a long-term care or skilled nursing facility • Strong communication and interpersonal skills • Ability to work effectively in a fast-paced environment
Preferred Experience / Skills: • Experience with electronic medical records and other healthcare technology • Knowledge of gerontological nursing principles and practices • Certification in gerontology or long-term care nursing (e.g. CNA, CLNC)
