LVN / LPN LTC/SNF

Job Title LVN / LPN - LTC/SNF Location Meeker, CO Schedule Nights Key Responsibilities • Provide high-quality patient care in a long-term care setting, including administering medications, monitoring vital signs, and performing various medical procedures. • Develop and implement individualized care plans for patients, in collaboration with interdisciplinary teams. • Conduct thorough assessments of patients' physical, emotional, and social needs, and make recommendations for care and services. • Educate patients and their families on proper wound care, medication management, and other health-related topics. • Maintain accurate and up-to-date records of patient information, including medical history, treatment plans, and progress notes.

Required Qualifications • Current, unrestricted LPN/LVN license • Experience working in a long-term care setting, preferably in LTC or SNF • Basic life support (BLS) certification • Ability to lift, transfer, and position patients as needed

Preferred Experience / Skills • Experience with Minimum Data Set (MDS) documentation • Knowledge of Medicare and Medicaid regulations and policies • Ability to work effectively in a fast-paced, dynamic environment
