LVN / LPN Home Health
Job Title LVN / LPN - Home Health Location Accomac, VA Schedule Full-time Key Responsibilities • Provide high-quality patient care in a home health setting, including administering medications, monitoring vital signs, and performing wound care • Develop and implement individualized care plans to meet the unique needs of each patient • Collaborate with interdisciplinary teams to coordinate patient care and achieve optimal outcomes • Educate patients and their families on proper care techniques and health promotion strategies • Maintain accurate and up-to-date patient records, including documentation of patient progress and treatment plans
Required Qualifications • Associate's degree in nursing (ADN) or Bachelor's degree in nursing (BSN) • Current licensure as a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) • 2 years of experience in home health care, with a focus on LPN/LVN duties • Proficiency in Epic system
Preferred Experience / Skills • Experience working with patients with chronic conditions, such as diabetes and heart failure • Knowledge of home health regulations and guidelines • Ability to work independently and as part of a team
