Key Responsibilities:
• Provide high-quality patient care to residents in a long-term care and skilled nursing facility setting • Develop and implement individualized care plans to meet the unique needs of each patient • Conduct vital signs, take medical histories, and perform other basic patient assessments • Administer medications and treatments as prescribed by physicians • Collaborate with interdisciplinary teams to coordinate patient care and achieve desired outcomes
Required Qualifications:
• Current licensure as a Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) • Current certification in Basic Life Support (BLS) and/or Cardiopulmonary Resuscitation (CPR) • Ability to communicate effectively with patients, families, and healthcare team members • Basic computer skills and ability to document patient information accurately
Preferred Experience / Skills:
• Previous experience working in long-term care or skilled nursing facilities • Knowledge of gerontological nursing principles and practices • Ability to work effectively in a fast-paced environment with multiple priorities and deadlines
