Key Responsibilities:
• Provide direct patient care under the supervision of an RN, including administering medications, changing wound dressings, and monitoring vital signs. • Develop and implement individualized care plans to meet the unique needs of residents, including managing chronic conditions and preventing complications. • Collaborate with interdisciplinary teams to coordinate care and ensure seamless transitions between services. • Educate patients and families on proper use of medical equipment, medications, and self-care techniques. • Maintain accurate and up-to-date records of patient care, including progress notes and medication administration records.
Required Qualifications:
• Current LPN/LVN license in the state of Delaware. • Basic Life Support (BLS) certification. • 1+ year of experience in long-term care or skilled nursing facility setting. • Strong communication and interpersonal skills.
Preferred Experience / Skills:
• Experience working with geriatric or post-acute populations. • Knowledge of medical terminology and coding systems (e.g. ICD-10). • Ability to lift 50 pounds and stand for long periods of time.
