Key Responsibilities:

• Provide skilled nursing care and health guidance to individuals and families in the home and community • Conduct comprehensive assessments of patients' physical, psychosocial, cultural, and environmental needs • Develop and implement individualized care plans, adjusting them based on patient progress • Coordinate care with interdisciplinary team members, including physicians, therapists, and aides • Educate patients and families on disease processes, self-care, prevention, and ethical concerns • Provide symptom management and specialized nursing interventions, including for hospice patients • Maintain accurate and timely clinical documentation • Participate in team conferences, quality assurance activities, and performance improvement initiatives • Serve as a mentor and resource to other staff, patients, and caregivers • Participate in an on-call rotation as required

Required Qualifications:

• Current Vermont Registered Nurse (RN) license • BSN preferred • Minimum of one year of clinical nursing experience (community nursing or related field preferred) • Home health experience required • Valid driver’s license and reliable transportation required • Strong computer proficiency and ability to maintain electronic health records • Excellent communication, organizational, and problem-solving skills • Ability to work independently and collaboratively in a dynamic community health environment • Experience with Oasis
