Key Responsibilities:
• Provide high-quality patient care to residents in a long-term care and skilled nursing facility setting • Develop and implement individualized care plans, including medication management and vital signs monitoring • Collaborate with interdisciplinary teams to coordinate patient care and achieve optimal outcomes
Required Qualifications:
• Current licensure as a Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) in the state of VA • Basic Life Support (BLS) certification • Ability to pass a background check and obtain any required facility-specific certifications
Preferred Experience / Skills:
• Previous experience in long-term care and skilled nursing facilities • Knowledge of medical terminology and charting systems • Strong communication and interpersonal skills
