Key Responsibilities • Provide direct patient care to residents in long-term care and skilled nursing facilities • Assess patient needs, develop care plans, and implement interventions to promote health and well-being • Collaborate with interdisciplinary teams to coordinate patient care and achieve optimal outcomes • Administer medications, perform vital signs, and document patient information accurately and efficiently
Required Qualifications • Current licensure as a Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) in the state of VA • Experience working in long-term care or skilled nursing facilities preferred • Strong communication and interpersonal skills
Preferred Experience / Skills • Experience working with geriatric populations or residents with complex medical needs • Familiarity with electronic medical records (EMRs) and other clinical software systems
