Job Title: LVN / LPN - LTC/SNF

Location Fredericksburg, VA

Schedule / Job Type Part-time, PRN Block contract

Key Responsibilities:

• Provide high-quality nursing care to residents in a long-term care and skilled nursing facility setting • Assess patient needs, develop individualized care plans, and implement evidence-based interventions to promote optimal health outcomes • Collaborate with interdisciplinary teams to coordinate patient care and ensure seamless transitions • Educate patients and families on disease management, medication adherence, and self-care techniques • Maintain accurate and up-to-date patient records, including medical charts and documentation

Required Qualifications:

• Current licensure as a Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) in the state of VA • 1+ year of experience in a long-term care or skilled nursing facility setting • Strong communication and interpersonal skills • Ability to work effectively in a fast-paced environment with multiple priorities • Basic life support certification (BLS)

Preferred Experience / Skills:

• Experience working with geriatric populations • Familiarity with electronic medical records (EMRs) • Strong critical thinking and problem-solving skills • Ability to work independently and as part of a team
