Key Responsibilities:
• Provide high-quality physical therapy services to patients in a variety of settings, including SNF and rehab facilities • Conduct comprehensive assessments and develop personalized treatment plans to address patient needs and goals • Implement evidence-based interventions, such as exercises, modalities, and education, to promote patient mobility, strength, and function • Collaborate with interdisciplinary teams to ensure comprehensive care and optimal patient outcomes
Required Qualifications:
• State License as a Physical Therapist • Current resume and license verification attached • Three professional references
Preferred Experience / Skills:
• SNF experience • Rehab Optima experience • Strong knowledge of evidence-based practice and current rehabilitation modalities
