Job Title: Therapist - Occupational Therapist

Schedule / Job Type: Full-time, Days
Key Responsibilities: • Provide occupational therapy services to patients, including assessments, treatment plans, and interventions. • Develop and implement individualized treatment plans to address patient needs and goals. • Collaborate with interdisciplinary teams to coordinate patient care and achieve optimal outcomes.
Required Qualifications: • State License as an Occupational Therapist • Current resume • References (3) • County Criminal (7 years for all counties lived and worked) • Federal Criminal (7 years or more) • Multi-State Criminal Search (7 years or more) • Nationwide Sex Offender Search (7 years or more) • HHS/OIG Search • GSA Search • Social Security Number Trace • 8 Hours of Dementia Training
Preferred Experience / Skills: • SNF experience preferred • Covid-19 vaccination requirements (proof of vaccination 72 hours prior to start date)
