Job Title: LVN / LPN - LTC/SNF
Location Lancaster, NH
Schedule / Job Type: Full-time - Days
Key Responsibilities:
• Provide high-quality patient care in a long-term care and skilled nursing facility setting • Conduct patient assessments, develop individualized care plans, and implement interventions to promote optimal health outcomes • Administer medications, treatments, and therapies as ordered by physicians and in accordance with facility policies and procedures • Collaborate with interdisciplinary teams to coordinate patient care and achieve desired outcomes
Required Qualifications:
• Current licensure as a Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) in the state of NH • Valid certification in basic life support (BLS) and/or other specialized certifications as required by the facility • Ability to communicate effectively with patients, families, and healthcare teams
Preferred Experience / Skills:
• Experience working in long-term care and skilled nursing facilities • Knowledge of medical terminology and anatomy • Ability to work independently and as part of a team in a fast-paced environment
