Registered Nurse - LTC/SNF

Schedule / Job Type Full-time
Key Responsibilities:
• Provide high-quality patient care in a long-term care setting, including administering medications, performing vital signs, and developing individualized care plans. • Collaborate with interdisciplinary teams to coordinate patient care and achieve optimal outcomes. • Monitor patient conditions, identify potential issues, and report concerns to supervisors or physicians as needed. • Educate patients and families on health promotion, disease prevention, and management of chronic conditions.
Required Qualifications:
• Current RN licensure in the state of NH • Minimum 1 year of experience in LTC or SNF setting • Basic Life Support (BLS) certification • Ability to lift, transfer, and position patients as needed
Preferred Experience / Skills:
• Experience with electronic medical records (EMRs) and other clinical software • Knowledge of gerontological nursing principles and practices • BSN degree preferred
