Job Title: LVN / LPN - LTC/SNF

Schedule / Job Type Full-time
Key Responsibilities: • Provide high-quality patient care in a long-term care or skilled nursing facility setting • Develop and implement individualized care plans for residents, ensuring their physical, emotional, and social needs are met • Administer medications, treatments, and therapies as prescribed by physicians • Conduct vital signs, take medical histories, and perform other basic patient care duties
Required Qualifications: • Current licensure as an LVN/LPN in the state of VT • Valid CPR certification • Basic life support certification
Preferred Experience / Skills: • Experience working in LTC/SNF settings • Knowledge of medical terminology and charting systems • Ability to work effectively with interdisciplinary teams
