Key Responsibilities • Provide high-quality patient care in a long-term care and skilled nursing facility setting • Conduct comprehensive patient assessments, develop and implement individualized care plans, and monitor patient progress • Administer medications, treatments, and therapies as prescribed by physicians • Collaborate with interdisciplinary teams to coordinate patient care and ensure seamless transitions
Required Qualifications • Current licensure as a Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) in the state of NM • Basic Life Support (BLS) certification • Ability to work effectively in a fast-paced environment and prioritize multiple tasks simultaneously
Preferred Experience / Skills • Experience working in long-term care and skilled nursing facilities • Knowledge of gerontological nursing principles and practices • Familiarity with electronic medical records (EMRs) and other healthcare technology
