Key Responsibilities: • Provide high-quality nursing care to residents in a long-term care setting, including administering medications, monitoring vital signs, and performing various medical procedures. • Develop and implement individualized care plans to meet the unique needs of each resident. • Collaborate with interdisciplinary teams to coordinate care and ensure seamless transitions. • Educate residents, families, and staff on various health-related topics and promote health literacy. • Maintain accurate and up-to-date records of resident care and progress.
Required Qualifications: • Current licensure as a Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) in the state of NM. • Basic Life Support (BLS) certification. • Ability to lift, transfer, and position residents as needed.
Preferred Experience / Skills: • Experience working in long-term care or post-acute care settings. • Knowledge of medical terminology and ability to communicate effectively with residents and families. • Ability to work independently and as part of a team.
