Key Responsibilities • Provide high-quality patient care in a long-term care setting, including administering medications, monitoring vital signs, and performing various medical procedures. • Develop and implement individualized care plans to meet the unique needs of residents, in collaboration with interdisciplinary teams. • Educate patients and families on disease management, wound care, and other health-related topics. • Collaborate with physicians and other healthcare professionals to coordinate patient care and ensure seamless transitions.
Required Qualifications • Current licensure as a Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) in the state of NM. • Basic Life Support (BLS) certification. • Ability to lift up to 50 pounds and stand for long periods.
Preferred Experience / Skills • Previous experience working in long-term care or skilled nursing facilities. • Knowledge of medical terminology and ability to read and interpret charts and reports. • Strong communication and interpersonal skills.
