Job Title: Therapist - Occupational Therapy Assistant
Location: Colfax, WA
Schedule: Full-time, Days
Key Responsibilities: • Provide occupational therapy services to patients, including assessments, treatments, and interventions to promote independence and participation in daily activities. • Develop and implement individualized treatment plans, in collaboration with the interdisciplinary team. • Conduct home visits, assessments, and evaluations to determine the patient's needs and develop strategies to address them. • Collaborate with patients, families, and healthcare professionals to identify and achieve treatment goals.
Required Qualifications: • State License as an Occupational Therapy Assistant (OTA) • Current resume • Resume verification • Three professional references • Negative county criminal background check (within the past 7 years) • Negative federal criminal background check (within the past 7 years or more) • Negative multi-state criminal search (within the past 7 years or more) • Negative nationwide sex offender search (within the past 7 years or more) • Negative HHS/OIG search • Negative GSA search • Social Security Number Trace • Washington State Background with Fingerprints (Facility-run)
Preferred Experience / Skills: • Experience working in SNF settings • Experience working with Rehab Optima • New graduate OTAs welcome to apply
