Key Responsibilities:
- Provide individualized nursing care to residents in accordance with physician orders and facility policies.
- Perform patient assessments, develop and implement personalized care plans.
- Administer medications, treatments, and injections accurately and safely.
- Monitor and document patient responses, physical and psycho-social status, and progress.
- Respond to emergency situations promptly, following established protocols.
- Collaborate with healthcare team members to ensure high-quality patient care and effective care transfers.
Required Qualifications:
- Valid California LVN or LPN license.
- Current BLS certification.
- Ability to perform manual skills required for patient care, including walking, bending, lifting, and procedures like blood draws and IV starts.
- Good communication skills in English, both verbal and written.
- Dependable with reliable attendance and punctuality.
- Strong assessment skills and ability to handle emergency situations effectively.
Preferred Experience / Skills:
- Previous work experience in LTC or SNF settings.
- Familiarity with electronic health records and Microsoft Office applications.
- Ability to work as part of a team and handle diverse patient needs.
