Key Responsibilities:

• Provide individualized nursing care to residents in accordance with physician orders and facility policies.
• Perform patient assessments, develop and implement personalized care plans.
• Administer medications, treatments, and injections accurately and safely.
• Monitor and document patient responses, physical and psycho-social status, and progress.
• Respond to emergency situations promptly, following established protocols.
• Collaborate with healthcare team members to ensure high-quality patient care and effective care transfers.

Required Qualifications:

• Valid California LVN or LPN license.
• Current BLS certification.
• Ability to perform manual skills required for patient care, including walking, bending, lifting, and procedures like blood draws and IV starts.
• Good communication skills in English, both verbal and written.
• Dependable with reliable attendance and punctuality.
• Strong assessment skills and ability to handle emergency situations effectively.

Preferred Experience / Skills:

• Previous work experience in LTC or SNF settings.
• Familiarity with electronic health records and Microsoft Office applications.
• Ability to work as part of a team and handle diverse patient needs.
