Key Responsibilities:

• Provide occupational therapy services to patients in an SNF setting.
• Complete thorough pre-screenings, credentialing, and documentation prior to patient engagement.
• Verify and maintain all required licenses, certifications, and clearances.
• Collaborate with healthcare teams to develop and implement personalized treatment plans.
• Ensure compliance with all state and federal healthcare regulations.
• Document patient progress and maintain accurate medical records.

Required Qualifications:

• Valid Indiana state license as an Occupational Therapist.
• CPR certification.
• Ability to pass background and credentialing checks, including state and federal criminal searches, sex offender registry, and social security verification.
• Must have or be willing to obtain COVID-19 test results within required timelines.

Preferred Experience / Skills:

• Prior SNF (Skilled Nursing Facility) experience preferred.
• Recent graduate therapists considered.
• Experience with physical and occupational therapy documentation standards.
