Job Title: Therapist - Speech Language Pathologist

Location: Longview, WA

Schedule / Job Type: Full-time, Days

Key Responsibilities:

• Provide comprehensive speech therapy services to patients in a Skilled Nursing Facility environment.
• Evaluate and develop treatment plans tailored to each patient's needs.
• Document patient progress and coordinate with healthcare teams to optimize outcomes.
• Ensure compliance with state and federal licensing and safety requirements.
• Maintain accurate records and complete all necessary pre-employment credentialing documentation.

Required Qualifications:

• Valid State Speech Language Pathologist license.
• Ability to pass required background checks, including criminal and sex offender searches.
• Recent COVID-19 test results (within 72 hours prior to start date) with documentation (within 48 hours of start date).
• Submission of a complete prescreen, resume, and license verification.

Preferred Experience / Skills:

• Experience working in Skilled Nursing Facilities (SNF) preferred.
• New graduates are welcome to apply.

Credentialing Requirements:

• State license verification.
• Three professional references.
• Criminal background checks (county, federal, multi-state, nationwide sex offender search, HHS/OIG, and GSA).
• Social Security number trace.
• Lung health screening (PPD, Quantiferon, or chest X-ray proof).
• Washington State background check with fingerprints (facility run).
