Registered Nurse LTC/SNF

Job Title: Registered Nurse - LTC/SNF

Location: Canby, MN

Schedule / Job Type: Mids shift, Full-time

Key Responsibilities:

• Provide nursing care to residents primarily with dementia in a long-term care or skilled nursing facility
• Monitor patient health status, administer medications, and document care using EMR systems
• Collaborate with the interdisciplinary team to develop and implement individual care plans
• Ensure compliance with all facility policies and regulatory standards
• Maintain clear and accurate records, with duties primarily focused on non-IV patient care

Required Qualifications:

• Valid active MN RN license
• Experience working with dementia or similar populations preferred
• Proficiency with EMR systems (Matrix preferred)
• Ability to work independently and as part of a team
• Willingness to work every other weekend and holiday shifts

Preferred Experience / Skills:

• Prior experience in long-term care or skilled nursing facilities
• Knowledge of non-IV patient care procedures
• Availability to work 8-hour shifts, with some 12-hour shift options
