Job Title: Physical Therapist
Location: Monterey, CA
Schedule / Job Type: Days, Full-Time

Key Responsibilities:

• Provide comprehensive physical therapy services to patients in a skilled nursing facility setting.
• Develop and implement individualized treatment plans to promote patient recovery and mobility.
• Collaborate with healthcare teams to coordinate patient care and ensure optimal recovery outcomes.
• Document patient progress and maintain accurate clinical records.
• Ensure compliance with all regulatory and safety standards.

Required Qualifications:

• California licensure as a Physical Therapist, currently active.
• At least 1 year of experience working in a Skilled Nursing Facility (SNF).
• Knowledge of NetHealth (Rehab Optima) or similar electronic health record systems.
• Strong clinical skills and ability to work independently.

Preferred Experience / Skills:

• Prior experience with SNF and familiarity with rehabilitation protocols.
• Ability to adapt treatment plans based on patient needs and progress.
• Excellent communication and teamwork skills.
