Job Title: LVN / LPN - LTC/SNF

Location: Middlebury, VT

Schedule / Job Type: Full-time, Mids

Key Responsibilities:

• Provide licensed nursing care in a long-term care or skilled nursing facility environment.
• Administer medications and treatments following facility protocols.
• Monitor and document patient conditions using EPIC electronic health records.
• Communicate effectively with a multidisciplinary team to ensure quality patient care.
• Assist with patient activities and ensure compliance with care plans.
• Support patients aged 60 and above, including those with dementia.

Required Qualifications:

• Valid LVN or LPN license in the state of Vermont.
• At least 1 year of clinical experience in LTC or SNF settings.
• Prior travel nursing experience preferred and required within the past 2 years.
• EPIC experience within the last 2 years is mandatory.
• Ability to work Mids shifts, including weekends and holidays as required.
• Reliable transportation; public transit is not accessible in this area.

Preferred Experience / Skills:

• Dementia-capable care certification.
• Experience working with an elderly patient population.
• Ability to work 2 out of 4 major holidays (Thanksgiving, Christmas, Christmas Eve, New Year’s Day).

Additional Information:

• Facility has 98 beds, with specialized long-term care units.
• No block schedules; flexible shifts requested.
• All employment gaps over 90 days must be explained.
• Complete work history including bed count, EMR, and facility type required for submission.
• Floating to other units or facilities may be required based on staffing needs.
• Unit utilizes EPIC for documentation and medication administration.
• Must be comfortable working with a team, including ancillary staff such as CNAs and unit clerks.
• Shift hours are biweekly, with an alternating schedule of 4 shifts of 8 hours and 5 shifts of 8 hours.
• Candidate must be able to work all holidays and perform circulating/scrubbing as applicable.
