Job Title: Therapist - Physical Therapy Assistant
Location: La Grande, OR
Schedule / Job Type: Day shifts, Full-time

Key Responsibilities:

• Provide physical therapy assistance under supervision, supporting patient rehabilitation plans
• Assist in implementing treatment procedures and documenting patient progress
• Collaborate with interdisciplinary teams to ensure quality care delivery
• Maintain cleanliness and proper operation of therapy equipment
• Adhere to facility protocols and safety standards

Required Qualifications:

• Valid State License as a Physical Therapy Assistant
• Ability to pass a COVID-19 test within 72 hours prior to start date, with results issued 48 hours before
• Prescreen completed and attached with application, including resume, license verification, and references
• Must meet credentialing checklist requirements: criminal background checks (7 years), social security trace, federal and state criminal searches, sex offender registry, HHS/OIG, GSA, and Oregon CRIMS background check

Preferred Experience / Skills:

• Prior experience in skilled nursing facilities (SNF) preferred
• Recent graduate acceptable
• Strong communication and team collaboration skills
