Job Title: Physical Therapist
Location: Brockton, MA
Schedule / Job Type: Full-time, Days

Key Responsibilities:

• Provide patient-centered physical therapy services in a skilled nursing facility setting
• Assess patient needs and develop individualized treatment plans
• Document therapy sessions and progress according to facility protocols
• Collaborate with healthcare team to ensure optimal patient outcomes
• Adhere to all credentialing, safety, and compliance requirements

Required Qualifications:

• Valid state license as a Physical Therapist
• Resume and license verification must be submitted with application
• Successful completion of prescreening process, including background checks and credentialing checklist
• Minimum of 7 years of criminal background checks across applicable counties and federal levels
• Proof of ongoing Continuing Education (such as Dementia Training)

Preferred Experience / Skills:

• Experience working in a skilled nursing facility (SNF) environment is highly preferred
• Prior experience with credentialing requirements and documentation
• Knowledge of compliance regulations and safety protocols in healthcare settings
