Key Responsibilities

• Manage a panel of behavioral health patients, providing clinical oversight, medication management, and support.
• Conduct patient assessments, monitor medication adherence and side effects, and track progress toward treatment goals.
• Facilitate communication between patients, physicians, and pharmacies to ensure coordinated care and medication refills.
• Provide patient and family education on diagnoses, treatments, and self-help strategies to promote wellness and recovery.
• Document services accurately and timely according to organizational policies and regulations.
• Support care planning through collaboration with primary care and specialty providers, ensuring comprehensive patient care.

Required Qualifications

• Active nursing license in Alaska or an eligible U.S. state; application for Alaska license must be submitted at hire.
• Alternatively, LPN license with 5+ years of experience in behavioral health.
• High school diploma or equivalent.
• Clinical competency within 3 months of hire and ongoing every three years.
• Basic Life Support (BLS) certification.

Preferred Experience / Skills

• At least 6 months of nurse case management experience.
• Experience in mental health, substance use disorder treatment, and primary care triage.
• Knowledge of psychiatric and SUD medications, disease processes, and healthcare systems.
• Effective communication skills with diverse patient populations and healthcare teams.
• Proficiency with electronic medical records and clinical technology.
