Key Responsibilities
- Manage and coordinate care for patients within the nursing department to promote quality and cost-effective healthcare outcomes.
- Collaborate with healthcare providers and interdisciplinary teams to establish individualized medical goals.
- Facilitate multi-disciplinary care conferences for high-risk or complex patients.
- Oversee health maintenance, acute, and chronic care needs by reviewing electronic health records and providing patient outreach and education.
- Track referrals, follow their progress, and ensure timely completion while serving as the primary liaison for patients, providers, and other case managers.
- Perform staff nursing duties, including assessment, documentation, medication administration, and patient education, maintaining safety and infection control standards.
- Participate actively in team efforts, contributing to department improvements and supporting emergency responses as needed.
- Facilitate specialty clinic referrals, manage waitlists, and coordinate diagnostic procedures and clinic scheduling.
- Act as the primary case manager for obstetric patients, tracking postpartum care and immunizations.
- Coordinate telehealth visits and manage referrals for exercise treadmill testing.
Required Qualifications
- Graduate of an accredited school of nursing (Bachelor’s degree preferred).
- Active and unrestricted RN license in Alaska or another U.S. state; must have applied for Alaska license prior to start date.
- Basic Life Support (BLS) certification.
- Clinical competency within 3 months of hire and ongoing every three years.
- Minimum of 1 year of general nursing experience, with specific experience in outpatient clinics or case management considered a plus.
- Knowledge of health services in Alaska and excellent understanding of nursing processes, safety, and infection control.
- Strong assessment, communication, and multi-tasking skills; ability to work independently in a fast-paced environment.
- Ability to lift and move up to 50 lbs and perform physical tasks required in clinical settings.
Preferred Experience / Skills
- Case management certification.
- Experience with specialty clinics and health education initiatives.
- Familiarity with telehealth coordination and obstetric care.
