Key Responsibilities

  • Collaborate with a consortium-wide team to support inpatient Care Coordination and Utilization Management.
  • Coordinate the local Community Acute Hospital (CAH) Swing Bed/Transitional Care program.
  • Lead interdisciplinary team meetings for inpatient care.
  • Review all patient admissions to develop personalized, clinically appropriate treatment plans aligned with patient needs, preferences, and available resources.
  • Ensure timely and appropriate discharges while complying with regulatory and payor criteria.
  • Collaborate with providers, leadership, and clinical support staff to ensure comprehensive treatment plans.
  • Maintain accurate documentation supporting Utilization Management functions and communicate effectively with payors.
  • Educate patients and families regarding healthcare benefits and their treatment plans.
  • Conduct patient assessments to gather psycho-social information, provide interventions, and facilitate referrals as needed.
  • Develop and communicate discharge plans ensuring smooth transitions of care.
  • Support the peer review process as part of the consortium plan.

Required Qualifications

  • Current, unrestricted Registered Nurse license in Alaska or another U.S. state.
  • Bachelor's degree preferred; must have applied for Alaska RN licensure before start date if hired directly.
  • Active Alaska RN license required for agency staff.
  • Case Management certification within 24 months of hire.
  • BLS certification; ACLS and PALS preferred.
  • High school diploma or equivalent required.
  • Clinical competency required within 3 months of hire and every three years.
  • High-Risk competency required within 3 months of hire and annually.

Preferred Experience / Skills

  • At least 5 years of clinical nursing experience.
  • One year of experience in chart review, risk management, or related quality services preferred.
  • Knowledge of medical record review, medical necessity/insurance guidelines, and regulatory compliance.
  • Strong verbal and written communication skills.
  • Excellent organizational abilities.
  • Ability to work effectively within an interdisciplinary team and build relationships with colleagues, community partners, and patients/families.

Physical Demands

  • Regularly required to talk and hear; frequently stand, walk, sit, and use hands for various tasks.
  • Occasionally required to climb, balance, stoop, kneel, crouch, or crawl.
  • Capable of lifting and moving up to 50 lbs.
