Key Responsibilities

• Provide nursing care to residents in a long-term care or skilled nursing facility setting
• Float between two floors as needed, supporting patient care across units
• Collaborate with healthcare team to ensure residents’ needs are met
• Follow established protocols and documentation standards for LTC/SNF environments
• Assist with implementing care plans and monitoring residents’ health status

Required Qualifications

• Valid LVN/LPN license
• Minimum of 2 years recent experience working in a long-term care or skilled nursing facility
• Ability to work night shifts and adapt to a floating schedule
• Proven competence in basic nursing skills relevant to LTC/SNF
• Strong communication and teamwork skills

Preferred Experience / Skills

• Experience working in multiple floors or units within a LTC/SNF setting
• Familiarity with PCC or similar electronic medical record systems
• Prior experience in a charge nurse role (not required but advantageous)
