Key Responsibilities
• Provide skilled physical therapy services to patients in a skilled nursing facility (SNF).
• Conduct initial evaluations and develop individualized treatment plans.
• Implement therapy programs and monitor patient progress.
• Document patient care and communicate with healthcare team.
• Ensure compliance with all regulatory and facility standards.
• Maintain accurate and timely documentation, including treatment notes and progress reports.

Required Qualifications
• Valid Virginia state license as a Physical Therapist.
• Flexibility to work the assigned days schedule.
• Ability to pass all required background checks and screenings.
• COVID-19 testing within 72 hours prior to start date, with results issued 48 hours before start.
• Prescreen completed and attached with submission, including resume and license verification.

Preferred Experience / Skills
• Previous SNF experience preferred.
• Recent graduate therapists are welcome to apply.
• Experience with prior credentialing processes and background checks.
