Key Responsibilities
• Provide outpatient physical therapy services to patients in diverse clinical settings
• Develop and implement individualized treatment plans to improve patient mobility and function
• Monitor patient progress and adjust treatments accordingly
• Document patient assessments and interventions accurately in compliance with regulatory guidelines
• Collaborate with healthcare team members to ensure optimal patient care
• Maintain knowledge of current therapy techniques and evidence-based practices

Required Qualifications
• Valid state license as a Physical Therapist
• Relevant degree in Physical Therapy from an accredited program
• Ability to pass credentialing checks, including criminal background and social security verification
• Experience with Rehab Optima software preferred

Preferred Experience / Skills
• Prior outpatient therapy experience preferred
• Strong communication and patient care skills
• Ability to work independently and as part of a multidisciplinary team
