Key Responsibilities:

• Provide physical therapy services to patients in skilled nursing facility (SNF) settings.
• Assess patient needs and develop individualized treatment plans.
• Implement therapeutic interventions to improve mobility, strength, and function.
• Document patient progress and update treatment plans accordingly.
• Collaborate with healthcare team members to ensure comprehensive patient care.
• Ensure compliance with all licensing, credentialing, and safety requirements.

Required Qualifications:

• Valid state licensure as a Physical Therapist.
• Must complete and submit prescreen form, resume, and license verification.
• Ability to provide references (minimum of 3).
• COVID-19 test results from within 72 hours prior to start date, with results issued within 48 hours.
• Willingness to work days.

Preferred Experience / Skills:

• Prior SNF experience preferred.
• New graduates are welcome to apply.
