LVN / LPN - Long-Term Care / Skilled Nursing Facility

Location: Tyndall, SD

Schedule / Job Type: Night shift, Full-time

Key Responsibilities:

• Provide nursing care to residents in a long-term care or skilled nursing environment during overnight hours.
• Monitor and assess patients’ conditions, administer medications, and document care accurately.
• Collaborate with the care team to ensure residents’ needs are met and safety protocols are followed.
• Follow facility-specific protocols and EMR documentation standards.
• Assist residents with activities of daily living as needed, including dementia and rehab patients.
• Comply with weekend and holiday staffing requirements.

Required Qualifications:

• Valid LVN or LPN license in the state of South Dakota.
• Experience with PCC or similar EMR systems preferred.
• Ability to work night shifts and adhere to schedule requirements.
• Strong patient assessment and communication skills.
• Ability to work independently and as part of a team.

Preferred Experience / Skills:

• Prior experience in long-term care or skilled nursing facilities.
• Knowledge of dementia care and rehabilitation patient needs.
