Key Responsibilities

• Coordinate utilization management activities across the hospital, serving as the primary link between patients, healthcare teams, and payors during inpatient stays.
• Conduct patient assessments to identify needs, plan resource transitions, and secure necessary services.
• Collaborate with interdisciplinary teams through care rounds, conferences, and planning to ensure quality patient care.
• Communicate effectively in written, verbal, and electronic systems, including email and hospital-specific software.
• Work with community organizations to arrange durable medical equipment, obtain health insurance authorizations, and coordinate transportation.
• Participate in hospital length-of-stay management and support quality improvement initiatives.

Required Qualifications

• Associate or Bachelor degree-prepared Registered Nurse.
• 3-5 years of clinical experience in medical/surgical nursing, home care, hospital case management, or utilization review.
• Experience with discharge planning and utilization review processes.
• Active BLS certification.
• Knowledge of hospital reimbursement methods and quality metrics.

Preferred Experience / Skills

• Certification from a nationally recognized case management or utilization review organization (e.g., CCM or ACM).
• Strong communication skills, both written and verbal.
• Ability to work independently and lead care coordination efforts.
