Key Responsibilities
- Coordinate utilization management activities across the hospital, serving as the primary link between patients, healthcare teams, and payors during inpatient stays.
- Conduct patient assessments to identify needs, plan resource transitions, and secure necessary services.
- Collaborate with interdisciplinary teams through care rounds, conferences, and planning to ensure quality patient care.
- Communicate effectively in written, verbal, and electronic systems, including email and hospital-specific software.
- Work with community organizations to arrange durable medical equipment, obtain health insurance authorizations, and coordinate transportation.
- Participate in hospital length-of-stay management and support quality improvement initiatives.
Required Qualifications
- Associate or Bachelor degree-prepared Registered Nurse.
- 3-5 years of clinical experience in medical/surgical nursing, home care, hospital case management, or utilization review.
- Experience with discharge planning and utilization review processes.
- Active BLS certification.
- Knowledge of hospital reimbursement methods and quality metrics.
Preferred Experience / Skills
- Certification from a nationally recognized case management or utilization review organization (e.g., CCM or ACM).
- Strong communication skills, both written and verbal.
- Ability to work independently and lead care coordination efforts.
