Job Title
• Registered Nurse - Nurse Care Coordinator

Location
• Española, NM

Schedule / Job Type
• Full-time, day shifts, on-site with floating to local locations within 20 miles. May include on-call, weekends, and holidays.

Key Responsibilities
• Assess and interview patients, families, and designated individuals within 48 hours of admission to determine comprehensive care and discharge needs.
• Coordinate and develop discharge plans in collaboration with interdisciplinary teams, patients, and families to ensure timely and effective transition of care.
• Communicate medical necessity documentation with physicians and escalate cases not meeting criteria to supervisors or medical directors.
• Educate patients, families, and healthcare team members on post-acute care options and care coordination services.
• Monitor length of stay, manage barriers to discharge, and participate in daily multidisciplinary rounds to optimize patient care and resource utilization.
• Document assessments, interventions, and discharge plans in accordance with departmental policies, ensuring seamless communication for ongoing care.

Required Qualifications
• Valid nursing license in the state of New Mexico or a Compact State.
• Bachelor’s Degree in Nursing (ADN or ASN accepted).
• BLS certification through the American Heart Association, American Red Cross, or American Health and Safety.
• Minimum two years of current experience, preferably in a specialized setting, with at least one completed 13-week travel assignment.
• Ability to work in a fast-paced environment, collaborative team player, flexible with floating responsibilities.

Preferred Experience / Skills
• Experience in care coordination, discharge planning, or case management preferred.
• Familiarity with inpatient and outpatient clinical social work practices and psychosocial assessments.
• Strong communication skills for multidisciplinary collaboration.
