Therapist - Physical Therapist
Location: Lebanon, OR
Schedule / Job Type: Full-time, Days, Outpatient
Key Responsibilities:

• Evaluate and treat patients with injuries or conditions affecting mobility or functional independence.
• Provide outpatient therapy services including orthopedic, sports, pediatric, neurological, stroke, balance and dizziness, wheelchair seating, and neck/back extremity care.
• Assist patients in achieving desired recovery outcomes through customized treatment plans.
• Collaborate with multidisciplinary teams to ensure comprehensive patient care.
• Maintain accurate documentation of patient progress and treatment plans.
• Travel to multiple points of care as required.

Required Qualifications:

• Current unencumbered Oregon Physical Therapy license.
• Healthcare Provider Level BLS certification.
• Ability to travel to different care locations.

Preferred Experience / Skills:

• Previous outpatient therapy experience in a clinical setting.
• Strong knowledge of orthopedic and neurological rehabilitation techniques.
