Key Responsibilities
- Deliver personalized, in-home nursing care to a diverse patient population.
- Provide services such as wound care, medication administration, catheter care, and more.
- Monitor and report changes in patient condition to the RN and care team.
- Support chronic disease management and educate patients and families.
- Maintain accurate and timely documentation.
- Travel locally within Mesa County, with mileage reimbursed.
Required Qualifications
- Active LPN license in the State of Colorado.
- Valid driver's license and reliable transportation.
- Strong clinical skills, organization, and communication abilities.
- Compassionate attitude and dedication to patient care.
Preferred Experience / Skills
- IV certification is a plus.
- Home health experience is preferred.
