Key Responsibilities

• Provide patient care by delivering selected treatments, procedures, tests, and basic nursing care as ordered by physicians, under the supervision of the RN.
• Collaborate with a multidisciplinary team to meet the physical, emotional, and psychosocial needs of hospice patients across varying age groups.
• Utilize knowledge of growth and development principles to ensure appropriate, individualized patient care.
• Maintain compliance with regulatory standards and assist the Care Manager with utilization practices.
• Document patient care accurately and promptly, demonstrating proficient use of medical and office equipment, including Point of Care devices.
• Participate in on-call duties, including nights, weekends, holidays, and make home visits as required.

Required Qualifications

• Current license as a Licensed Practical Nurse (LPN) in Illinois.
• CPR certification prior to providing patient care.
• Valid driver’s license, with own transportation and auto insurance.
• Ability to work independently, prioritize tasks, and manage own workload.
• Competence in the nursing process and ability to collaborate effectively with team members.
• Willingness to work flexible hours, including night and weekend shifts.

Preferred Experience / Skills

• One year of recent nursing experience, preferably in home health care or hospice.
• Prior experience working in a clinical or community-based setting.
