Key Responsibilities
- Provide comprehensive diabetes education and management support to patients with various forms of diabetes in clinic, telehealth, and in-village settings.
- Conduct in-person, telephonic, and video conferencing education sessions, including village travel as needed.
- Order laboratory tests in accordance with hospital policies and document all educational activities in electronic health records.
- Collaborate with village clinics, hospitals, and tribal health staff to promote diabetes care standards and health initiatives.
- Assist in planning and executing diabetes-related health promotion activities within the region, including support groups, media campaigns, and outreach events.
- Maintain and update the diabetes registry in partnership with the Diabetes Coordinator, ensuring compliance with grant requirements.
- Work closely with the interdisciplinary team, including dietitians and pharmacists, to ensure holistic patient care.
- Participate in departmental planning, goal setting, and community engagement to advance patient outcomes and organizational objectives.
Required Qualifications
- Associate's Degree in Nursing from an NLN-approved institution; BSN or MSN preferred.
- Valid registered nurse license in the State of Alaska in good standing.
- Minimum of 2 years of clinical health experience preferred.
- Demonstrated collaboration skills across disciplines.
- Knowledge of diabetes management and public health.
- Ability to operate medical equipment such as glucometers and scales.
- Willingness to travel via small aircraft and work in village, hospital, and clinic environments.
- Strong cross-cultural communication skills; bi-lingual in Yup’ik and English preferred.
- Basic Life Support (BLS) certification preferred.
- Ability to operate PC computers and electronic health record systems.
Preferred Experience / Skills
- Background in Diabetes Education.
- Experience working in rural or remote community health settings.
- Familiarity with grant programs and health initiatives targeting diabetes prevention and management.
