Full-time, 13-week contract, days shift (Monday-Friday, 9 AM - 5 PM)
Key Responsibilities
- Provide skilled physical therapy services to patients in a rehabilitation and geriatric setting
- Develop and implement individualized treatment plans
- Monitor patient progress and adjust therapy programs accordingly
- Document patient evaluations, treatment sessions, and progress notes
- Collaborate with healthcare team to coordinate patient care
Required Qualifications
- Valid Colorado Physical Therapist license
- Minimum of 1 year recent experience in a rehab or geriatric setting
- Strong understanding of physical therapy techniques and protocols
- Excellent communication and patient management skills
Preferred Experience / Skills
- Experience working in a long-term care or skilled nursing facility
- Ability to work independently and as part of a team
