- Full-time, shift: Midnights (Mids)
Key Responsibilities
- Provide resident care in a long-term care or skilled nursing setting, including dementia-capable services.
- Administer medication and treatments using the EPIC system.
- Monitor patient health, respond to patient needs, and document care accurately.
- Collaborate with interdisciplinary teams and assist with unit activities as needed.
- Float across designated units, ensuring continuity of care during assigned shifts.
- Adhere to facility protocols and safety standards, including infection control and documentation.
Required Qualifications
- Valid LVN / LPN license in the state of Vermont.
- Minimum of 1 year of clinical experience, with experience in long-term care or SNF preferred.
- Experience with EPIC electronic medical records within the past 2 years.
- Ability to work flexible shifts, including holidays if required.
- Reliable transportation, as public transit is not accessible in this area.
- Willingness to work 2 out of 4 major holidays (Thanksgiving, Christmas, Christmas Eve, New Year’s Day).
- Completion of a background check (7 years) including nationwide, county court, FACIS3, and sex offender searches.
Preferred Experience / Skills
- Dementia-capable care certification preferred but not required.
- Previous travel or float experience in a healthcare setting.
- Ability to meet all scheduled shift requirements and coordinate with staff.

- No block scheduling; schedule varies with no guaranteed weekends or days off unless specified in the contract.
- Job requires working on a rotating schedule, including occasional/ as-needed shifts.
