Job Title
• LVN / LPN - LTC/SNF

Schedule / Job Type
• Night shifts, variable hours, 8- or 12-hour shifts

Key Responsibilities
• Provide basic nursing care to long-term care and skilled nursing patients.
• Monitor and document patient conditions using electronic medical records.
• Assist with patient assessments and coordinate care with healthcare team.
• Follow facility policies for patient safety and infection control.
• Support patients with activities of daily living and ensure comfort.
• Come prepared for building orientation 1 hour before each shift and shadow team members as needed.

Required Qualifications
• Valid LVN or LPN license.
• Experience with geriatric patients in LTC or SNF settings preferred.
• Ability to work night shifts and adapt to variable schedules.
• Proficiency with Point Click Care or similar EMR systems.
• Strong communication and patient care skills.

Preferred Experience / Skills
• Prior experience in a skilled nursing facility or long-term care environment.
• Knowledge of geriatric care and common chronic conditions.
