Key Responsibilities
- Provide hospice nursing care to a caseload of geriatric patients in their home environments.
- Conduct two patient admissions per day, coordinating care and assessments efficiently across Thurston, Mason, and Lewis counties.
- Document all patient interactions and care activities using HomeCare HomeBase and Oasis systems, ensuring timely and accurate records.
- Maintain compliance with all healthcare regulations, documentation standards, and safety protocols.
- Communicate effectively with patients, families, and interdisciplinary teams to ensure quality end-of-life care.
Required Qualifications
- Valid Washington state RN license or a valid compact license.
- At least 1 year of recent skilled home health experience within the last 3 years.
- Current BCLS certification.
- Own reliable transportation for local travel between patient locations.
Preferred Experience / Skills
- Experience providing hospice care with a focus on geriatric populations.
- Ability to handle a typical caseload of up to 6 patients per day with productivity standards.
- Strong organizational skills to manage documentation and coordinate care effectively.
