Key Responsibilities

• Provide hospice nursing care to a caseload of geriatric patients in their home environments.
• Conduct two patient admissions per day, coordinating care and assessments efficiently across Thurston, Mason, and Lewis counties.
• Document all patient interactions and care activities using HomeCare HomeBase and Oasis systems, ensuring timely and accurate records.
• Maintain compliance with all healthcare regulations, documentation standards, and safety protocols.
• Communicate effectively with patients, families, and interdisciplinary teams to ensure quality end-of-life care.

Required Qualifications

• Valid Washington state RN license or a valid compact license.
• At least 1 year of recent skilled home health experience within the last 3 years.
• Current BCLS certification.
• Own reliable transportation for local travel between patient locations.

Preferred Experience / Skills

• Experience providing hospice care with a focus on geriatric populations.
• Ability to handle a typical caseload of up to 6 patients per day with productivity standards.
• Strong organizational skills to manage documentation and coordinate care effectively.
