Key Responsibilities
- Provide licensed practical nurse (LPN) services in a long-term care (LTC) or skilled nursing facility (SNF) setting.
- Assist with resident assessments, care planning, and medication administration.
- Collaborate with interdisciplinary team to ensure quality resident care.
- Complete MDS documentation and assessments (preferred).
- Ensure compliance with all state and facility regulations.
Required Qualifications
- Valid and unrestricted LPN or LVN license in Colorado.
- Prior experience working in a long-term care or skilled nursing facility.
- Strong understanding of resident care procedures.
- Ability to work independently and as part of a team.
Preferred Experience / Skills
- MDS experience (preferred).
- Knowledge of LTC/SNF operational protocols.
