Therapist - Physical Therapist

Schedule / Job Type: Days, Full-time, Onsite

Key Responsibilities:

• Conduct comprehensive assessments of residents’ physical abilities, movement limitations, and pain levels.
• Develop and implement personalized treatment plans based on evaluations, medical history, and patient goals.
• Provide evidence-based physical therapy interventions to enhance strength, coordination, balance, and mobility.
• Collaborate with physicians, nursing staff, and interdisciplinary team members to coordinate comprehensive patient care.
• Monitor and document patient progress, adjusting therapy plans as needed in accordance with regulatory guidelines.
• Educate patients and caregivers on mobility safety, home exercises, and equipment use.
• Ensure compliance with all regulatory documentation requirements and facility policies.
• Supervise physical therapist assistants and aides as needed.

Required Qualifications:

• Graduate of an accredited Physical Therapy program.
• Current state license to practice as a Physical Therapist.
• CPR certification (preferred or required depending on the facility).
• Excellent communication, documentation, and interpersonal skills.
• Ability to work independently and collaboratively within a multidisciplinary team.

Preferred Experience / Skills:

• Prior experience in a Skilled Nursing Facility or geriatric rehabilitation setting.
