LVN / LPN LTC/SNF
Job Title
- LVN / LPN – Long-Term Care / Skilled Nursing Facility

- Berlin, VT
Schedule / Job Type
- Full-time, Days shift, 16-32 weeks assignment, travel and local options available
Key Responsibilities
- Provide nursing care to residents in a long-term care or skilled nursing setting
- Float to different units as needed, including dementia care populations
- Work every other weekend and a minimum of 2 out of 4 major holidays (Thanksgiving, Christmas, Christmas Eve, New Year’s Day)
- Follow assigned schedules and work within facility policies
- Communicate patient needs and assist with treatment plans
- Complete all required documentation accurately and timely
Required Qualifications
- Valid LVN / LPN license and certification (BLS through AHA)
- Minimum of 3 years’ relevant nursing experience
- Prior travel nursing experience preferred
- Experience in LTC or SNF environments mandatory
- Willing to float between units and handle dementia patient populations
- Reliable transportation and access to all-season or snow tires during winter months
Preferred Experience / Skills
- Experience working with dementia care
- Comfortable working independently and adapting to multiple units
- Strong communication skills and attention to detail
